조기난소부전 환자에서 비타민 D 결핍, 염증 표지자와 난소 기능부전의 연관성

Association between Vitamin D Deficiency, Inflammatory Markers and Ovarian Dysfunction in Patients with Premature Ovarian Insufficiency

Premature ovarian failure(POI) is the loss of normal ovarian function before the age of 40, and the cause is mostly unknown. Immune dysregulation has been suggested as one of the causative mechanisms, and some studies have reported that inflammatory markers were increased. Vitamin D has a role in bone metabolism and also immune modulation. This study was aimed to analyze the association between vitamin D deficiency, inflammatory markers, ovarian dysfunction in POI women. Serum vitamin D and inflammatory markers such as leukocyte count, neutrophil to lymphocyte ratio(NLR), platelet to lymphocyte ratio(PLR), and red cell distribution width to platelet ratio(RPR) were evaluated in POI women and healthy control. Vitamin D deficiency was observed in 88.1%. Serum 25(OH)D levels of POI was 18.91±7.27 ng/ml significantly lower than control(p=0.01). BMD of femur and lumbar spine were also significantly lower than control. Women with POI had higher PLR than those of controls(p=0.04). Leukocyte count, NLR, and RPR were similar between two groups. Vitamin D levels were correlated with body weight(r=0.52, p=0.01), however, not correlated with FSH, E2, AMH, or inflammatory markers. Therefore, vitamin D deficiency showed a high prevalence in POI women, but it did not seem to affect estrogen deficiency or inflammatory markers.
