This study was conducted targeting radiation safety officers in the fields of education, research, and industry to identify operational issues and develop improvement measures to ensure safety in organizations that utilize radiation sources and radiation-generating devices. In the past five years, there have been 222 reported radiation accidents involving notified institutions, which averages more than 0.85 report every weeks. This highlights the urgent need for improvements in the notification system. The analysis of the survey results indicates that there are issues in safety management, such as the lack of reporting, follow-up, and inventory management after the initial notification. The handling of equipment by individuals who have not received radiation safety training was reported to be high. The survey results on improvement measures indicated that the most common response, accounting for 28%, was the necessity of completing initial and regular safety training to ensure the safety of workers directly handling radiological equipment. Based on these findings, it is essential to ensure the safe handling and management of sealed radioactive sources, which pose a high risk of contamination, exposure, and potential public concern. At the same time, it is necessary to enforce compliance with minimum regulatory standards as a basic obligation. It is suggested that safety in the use of radioactive sources can be ensured by implementing a system that requires institutions to submit and obtain approval for safety management regulations tailored to their operational characteristics, provide user training, conduct annual inventory reporting to monitor material integrity, and establish a validity period and renewal process for notifications.
