Clinical Analysis of Intraocular Pressure Changes after Scleral Buckle Surgery for Rhegmatogenous Retinal Detachment

Purpose: This study analyzed changes in intraocular pressure (IOP) and factors contributing to IOP elevation following scleral buckle (SB) surgery in patients with rhegmatogenous retinal detachment (RRD). Methods: A retrospective review was conducted on 152 eyes of 146 patients diagnosed with RRD who underwent SB surgery between January 2016 and February 2023. Postoperative IOP was measured, and factors influencing IOP elevation were analyzed. Results: The mean baseline IOP was 14.6 ± 3.1 mmHg, which increased significantly to 17.6 ± 4.8 mmHg at 1 day and 15.8 ± 4.0 mmHg at 2 days postoperatively (p < 0.001 for both). The mean change in IOP was 4.4 ± 3.8 mmHg. The baseline IOP (β = -0.388, p < 0.001) and buckling area (β = 0.346, p < 0.001) were significantly associated with postoperative IOP changes in univariate and multivariate analyses. Conclusions: IOP elevation may persist for up to 2 days following SB surgery. Factors such as lower baseline IOP and wider buckling area significantly influence postoperative IOP changes.
