Optimized AED Deployment Considering Vertical Isolation in High-Rise Residential Buildings

Survival rates of Out-of-Hospital Cardiac Arrest patients in high-rise residential buildings are significantly reduced due to vertical isolation. Existing AED deployment guidelines consider only two-dimensional factors, such as horizontal distance or the number of households, and do not account for vertical travel time caused by elevator waiting or stair climbing. This study introduces a three-dimensional spatio-temporal accessibility concept and proposes an optimal AED placement using a K-means clustering algorithm based on simulations of horizontal and vertical movements, applied to real data from a large apartment complex. Results indicate a statistically significant negative correlation between residential floor level and survival probability. Compared to conventional placement of AED only in the first-floor lobby, the proposed distributed deployment strategy substantially reduced defibrillation arrival times, markedly improving the expected survival rates of high-floor residents. The study provides a practical methodology for AED placement optimization in high-density urban environments and demonstrates its potential to enhance emergency medical response systems.
