Capsule endoscopy is usually a safe procedure. However, capsule retention is a complication that should be cautiously considered. Capsule retention is defined as the retention of the capsule in the gastrointestinal tract for at least 2 weeks. The presence of a combination of symptoms including abdominal pain, abdominal distension, and nausea or vomiting before capsule endoscopy has been found to be related to a significantly higher rate of capsule retention. However, capsule retention is usually asymptomatic. The capsule can be retained in the small bowel without symptoms for several months, and capsule retention can be naturally resolved during subsequent follow-up. When capsule retrieval is required, both device-assisted enteroscopy and surgery can be performed. Device-assisted enteroscopy could be the first-choice method. If surgical treatment is required or device-assisted enteroscopy is unsuccessful, capsule retrieval by surgery is indicated. Particularly, surgical intervention is primarily indicated in all cases in which the presence of neoplastic disease is suspected. The patency capsule, which could check for small-bowel stricture before capsule endoscopy examination, has been developed. Patency capsule testing is less likely to expose the patient to ionizing radiation and has a high negative predictive value. However, in some cases, bowel obstruction occurred after patency capsule ingestion. © Springer Nature Singapore Pte Ltd. 2022.
