A Case of Pneumocephalus Caused by Ethmoid Sinus Osteoma

Osteomas are slow-growing benign bone tumors. Pneumocephalus is an extremely rare complication associated with ethmoid osteoma. We describe a case of a patient with pneumocephalus caused by an ethmoid sinus osteoma, which was treated by endoscopic sinus surgery. A 49-year-old woman visited the emergency room with Broca’s aphasia. Paranasal sinus computed tomography (CT ) images revealed an ethmoid sinus osteoma extending to the skull base from the left ethmoid sinus roof, causing pneumocephalus. It was suspected that the pneumocephalus was due to an injury to the skull base caused by osteoma, and an endoscopic sinus surgery was planned to remove the osteoma. The surgery was completed after multilayer reconstruction of the defect. The aphasia improved 3 days after surgery, and normal conversation was possible at postoperative 5 days. After surgery, she was discharged without any complications, including cerebrospinal fluid leakage, and the pneumocephalus showed improvement on a CT scan taken after 3 months.
